REPEAT PRESCRIPTION SERVICE

If your doctor has decided that you should have a repeat prescription, it means that you do not have to see your doctor every time you need a prescription to be renewed.

You will be required to submit the tear-off slip or a written request to the surgery for your next repeat prescription.  Telephone requests will not be accepted.

Repeat prescriptions are subject to annual or 6 monthly review (your doctor will advise which is appropriate to you), please make an appointment with your doctor when your review is due so that your medications can be assessed.  This is to ensure your treatment is appropriate to your care.

Repeat prescriptions can be obtained in the following ways:

• order online by clicking on your practice online order link below
• tick the item(s) required on the tear off slip attached to your last prescription and bring the slip to your surgery,
• you may post your prescription request. Please provide a stamped addressed envelope and we shall be happy to post your prescription back to you,
• Faxed requests will be accepted. Please write the name of your doctor on the request.

Telephone requests will not be accepted.

Allow 48 hours (2 working days) before collecting your prescription from the surgery.  You also have the choice to have your prescription sent to a local chemist.

Alternatively you may wish to use the Prescription Collection Service offered by your local pharmacy. They will advise you what to do.

The local chemists in West Byfleet, Byfleet, New Haw, Pyrford and Sheerwater (Boots, Lloyds, Assura, Alliance, Healthcare and May & Thompson) operate repeat prescription services.

Prescriptions for Controlled Drugs.  Prescriptions for controlled drugs must be collected either by the patient (or patient's guardian, representative or nominated chemist staff) each of these would be required to sign and acknowledge receipt of the prescription.

Keep your repeat prescription request in a safe place, and if you go into hospital take it, as well as your medicines, with you.

Occasionally, your doctor will want to review your repeat prescription and may add or remove items, as appropriate.

• DO NOT OVER ORDER MEDICINES.
• REQUEST ONLY THE ITEMS NEEDED.
• DO NOT HOARD MEDICINES AT HOME AS IT IS UNSAFE AND RESULTS IN CONSIDERABLE WASTAGE AND THEREFORE EXPENSE TO THE NHS.
• IF YOU CHOOSE NOT TO TAKE ANY OF YOUR MEDICINES, PLEASE TELL YOUR DOCTOR.
