HOW TO MAKE YOUR APPOINTMENT

ON THE DAY ASSESSMENT

Telephone assessment - call reception (336933) if you have a problem needing attention 'on the day'. The duty doctor will call you back and assess your needs over the telephone. The doctor will then arrange treatment or an appointment as appropriate. Please provide a contact number which you will be available on when the doctor calls you back. Telephone as early as you can so that the duty doctor can prioritise their work load. The practice telephone liines open at 8.30am.

ROUTINE APPOINTMENTS

• In person - at reception
• Telephone - 01932 336933 (open 8.30am-6.30pm Mon-Fri)

Bookable up to 4 weeks in advance for GPs and 6 weeks in advance for nurses.

Please book as you leave the surgery (or as soon as possible) if the doctor or nurse asks to see you again.

We endeavour to book your doctor of choice but if this is not possible we will offer an alternative doctor.

Extended opening hours - routine appointments are available; late evening Tuesday (between 6.30-8pm) and early morning Friday (between 7-8am)

Cancelling your appointment - either in person or by phone; please remember to cancel with as much notice as possible. GP appointments can also be cancelled using the automated telephone system. Please listen carefully to the instructions. Occasionally emergency and complicated consultations can delay your doctor or nurse but it is important that you arrive promptly for your appointment. If you are late in arriving you may be asked to re-book.

HOME VISITS

Week days for housebound patients or those too ill to attend the Practice, telephone 01932 336933 before 10am if at all possible. The duty doctor will usually call before visiting to discuss your concern.
