CLINICS AND SERVICES WE PROVIDE

 

Through Practice Based Commissioning, the Health Centre can develop new services specifically for our population needs. Currently, we offer a Gynaecology service, Pelvic and Abdominal Ultrasounds, a Menopause service, Echocardiography, 24 hour Blood Pressure recording, Atrial Fibrillation service and a Minor Operations service. The practice offers a full range of health and disease management services including Ischaemic Heart Disease, Cardiovascular Disease, Chronic Renal Failure, Atrial Fibrillation and Hypertension, Diabetes, Asthma and COPD.

As Primary Care evolves we are dedicated to providing the relevant clinics for our patient needs.

 

Antenatal/Postnatal
Our GPs run antenatal clinics with the Community St Peters' Midwife, to ensure the best possible outcome for mother and baby. Our Midwife and Health Visitors hold parent-craft classes at the Health Centre.

 

Anticoagulation
We provide the ongoing surveillance of Warfarin therapy after initial stabilisation by the hospital.

 

Asthma / COPD
We provide a service which follows locally agreed protocols for community managed respiratory disease. This is run by the Practice Nurse and doctor with a special interest in respiratory disease. Treatment is constantly improving. We offer regular review of treatment and instruction in inhaler use and self help. Your doctor or nurse will advise which patients should attend.

 

Blood Pressure - 24 hour monitoring
We provide ambulatory blood pressure monitoring providing a more precise estimate of blood pressure over a 24 hour period.

 

Blood Tests
In addition to the blood test service provided in our Treatment Room, we provide specialist blood tests for certain conditions.

 

Cervical Smears

Women aged between 25 and 49 will be invited by letter to have a smear test every three years and women aged between 50 and 64 every five years. Tests are carried out by appointment by the Practice Nurse.

Chlamydia screening is available for 18-25 year old males and females.

 

Child Health
These are run by Health Visitors and 6 week postnatal check by GPs with special interest in child health. You may attend any time the clinic is open. An appointment is not needed. A Health Visitor is available to weigh your baby and discuss baby's progress and any problems you may have.

Immunisations - It is vital that all children are protected from infectious diseases. Please ask your Health Visitor for details or speak to a Practice Nurse.

 

Community Matron Service
The Community Matron Service, also provided by Surrey PCT, provides support to patients aged 65+ with long term conditions. The Community Matron Team are experienced in the active management of long term conditions. The Community Matron will support patients and carers to manage their condition and work in co-ordination with other Health and Social Care professionals to ensure you receive the best care and remain healthy and well at home. Referrals to the Community Matron Service can be made by your GP and other Health & Social Care services.

 

Contraceptive Services
A full range of services is offered including Mirena and copper coils, Implanon and contraceptive caps during normal surgery appointments; please ask your doctor or practice nurse for advice. Emergency contraception is available after discussion with a GP or nurse or through the local Walk-In Centres.

Emergency contraception service is available via pharmacists.

 

Counselling
Referral is by the doctor. Appointments are sent directly to the patient by the Brief Intervention and Counselling Service based at Woking Community Hospital. Counsellors are here to help you to work through a wide range of personal and family difficulties. The service provided is strictly confidential between the counsellor and the client.

 

Cryotherapy
Treatment for superficial skin lesions including benign skin lesions.

 

Diabetes
The nurses and doctors at the Health Centre work closely with the specialist diabetic clinic at St Peter's Hospital. All patients with diabetes on our register are invited for annual checks. We encourage patients to attend these reviews as good control of diabetes helps to avoid long-term complications.


District Nursing Service
The District Nursing service provided by Surrey Primary Care Trust is responsible for nursing care to patients in their homes or residential homes in the community. The District Nursing Sister is responsible for (a) assessment and review of the needs of each individual patient and carer and (b) provision of an appropriate care plan. Referrals to the District Nursing Service may be made either by your doctor, hospital or self referral.

District Nurses also run nurse led clinics at New Haw Day Centre and St Mary's Day Centre. Leg ulcer and Well Leg Clinics are also run by the District Nursing Service.

Health Visitors
This team, which comes under Surrey Primary Care Trust, takes care of children under the age of 5 and the elderly. All team members are qualified nurses with specialist training in family health, child development and the elderly. Their primary responsibility is for the promotion of good health and prevention of ill health. They may be contacted on 01932 340411. A Health Visitor is usually in the office between 09.00 and 10.00 and 16.00 and 17.00; otherwise a message may be left with the Health Visitor's secretary or receptionist 01932 356812. Our Health Visitors work closely with our midwife providing parenthood classes. Their prime function is to promote good health through education and to give parents an understanding of their child's development. Apart from child health clinics, they also carry out routine developmental checks on pre-school children.

 

Midwife
We have a fully qualified Community Midwife providing maternity care. She organises parenthood relaxation classes for parents-to-be. Mothers are seen at the antenatal clinics and visited at home in the first 10 days following delivery.

 

Minor Surgery
For non-cosmetic skin lessions that require removal under local anaesthetic.

 

New Patient Checks
We advise all new patients to make an appointment with the Health Care Assistant or Practice Nurse for a new patient check, so we can obtain details of your medical history and current medication, as it can take time before we receive your records from your previous doctor.

 

Podiatry
This service is via self referral (Telephone 01932 570607). Appointments are sent directly to the patient by the Podiatry Service at Chertsey Health Centre. Clinics are held at West Byfleet Health Centre.

 

Practice Nurses
We have a team of very experienced nurses, who work as part of the practice team in providing care. They run nurse-led services for asthma, chronic obstructive pulmonary disease, diabetes, coronary heart disease and hypertension (high blood pressure). They also provide childhood immunisation and travel health services, suture removal plus general health checks for men and women - including lifestyle advice on weight management, smoking cessation and cervical smears. Appointments are made by contacting your reception by appointment.

 

Sexual Health
If you are concerned about sexually transmitted infections you may wish to contact the Genito-Urinary Clinic at the Blanche Heriot Unit on 01932 722669 (Office), or 01932 722390 (Advice).

 

Smoking Cessation
If you are interested in giving up smoking, please book an appointment with a Practice Nurse or enquire at reception, where information about support groups is available.

 

School Nurses
The school nurses are part of the Community Health Services and are based in Woking Community Hospital. They visit local schools, monitoring children's health and providing advice to parents and children on relevant matters.

 

Specialist Medical Equipment
The Health Centre is uniquely equipped with an ultra-sound machine, which enables echocardiography, abdominal and pelvic examinations to be carried out without referral to a hospital. Specialist Nurses
A Macmillan Cancer Nurse is attached to the practice and meets regularly with the practice.

 

Travel Health
